I am 99% sure I never used the Funimation digital code, and I’m unsure if you can still claim it with Crunchyroll.

As for the damage, the digibook has a few issues. There are some light scratch marks on 2nd and 3rd page; it came like that. (see photos 9 & 10)
And the book cover is slightly askew because it got squished in my last move (though I imagine you can fix that by putting something heavy on it for a bit). (See photos 11 & 12).
